Robust on Paper, Leaky in Practice

I've sat across the table from leadership teams genuinely proud of a process, hiring, onboarding, delivery, and equally puzzled by why results keep slipping anyway. The pattern is almost always the same: the process is fine. The judgment applied at the final decision point isn't.
